Community Health Worker (Sign-on Bonus)

About the role

Activate Care is building a team of hybrid Care Coordinators in Nevada to support the Path Assist program, which helps people with health-related social needs. The role works from home and also requires commuting to clients' homes and field locations within about 30 minutes of Clark County, Las Vegas. It covers screening, assessment and care navigation for community members enrolled in the program.

Key responsibilities

• Navigate care and resources for a caseload of community members with unmet social needs
• Conduct telephonic and face-to-face outreach, follow-up and coaching to assess needs, give education and help with enrollment in eligible services and benefits
• Administer SDOH screening, intake forms and assessments in the Activate Care platform
• Help clients set goals and create client-centered care plans, and coordinate with nonprofits and community resources
• Improve health literacy and self-sufficiency, and support Medicaid and health plan navigation including appeals
• Coordinate care across settings and after hospital discharge or ER visits to prevent readmissions
• Support high-risk members facing housing instability or complex medical conditions
• Take a proactive approach to cases (appointments, applications, reminders) and protect client privacy
• Attend weekly team meetings, workshops and training

Required profile

• High school diploma or equivalent
• Active Certified Community Health Worker I (CHW I) credential through the Nevada Certification Board, or willingness to obtain it after hire
• Valid Nevada driver's license and ability to use a personal vehicle to reach clients
• 2-3 years of direct care coordination experience preferred
• Experience with nonprofits, social service providers, faith-based groups or government agencies addressing social determinants of health
• Ability to commute to and from client homes and field locations
