Physical Therapist – Home Health (Full‑time)

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide home health visits for adult and geriatric clients in senior living communities throughout Forsyth County, North Carolina. This full‑time position involves delivering rehabilitative care and collaborating with a multidisciplinary team.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests, mobility assessments, balance testing, and safety evaluations.
• Develop and implement individualized care plans, including therapeutic exercises, gait training, and wound‑care interventions.
• Educate clients and families on rehabilitative activities and document all observations using a tablet‑based EMR system.

Required profile

• Current North Carolina Physical Therapist license.
• Minimum of one year recent clinical experience as a licensed Physical Therapist.
• Graduation from an APTA‑approved program.
• Ability to work independently, manage time effectively, and possess strong interpersonal and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation on a touch‑pad tablet.
