Physical Therapist – Home Care

About the role

Join the Cleveland Clinic Health System as a Home Care Physical Therapist, providing patient‑first therapy in the comfort of their homes. You will work Monday through Friday, with occasional weekend shifts, delivering strength, range‑of‑motion, and endurance programs while building strong patient relationships.

Key responsibilities

• Evaluate and conduct medically prescribed physical therapy treatment programs.
• Perform initial and ongoing assessments, establish and revise plans of care.
• Deliver interventions using standard physical therapy techniques appropriate for the patient’s age and condition.
• Supervise physical therapy assistants, students, and support caregivers.
• Maintain complete, up‑to‑date documentation of patient condition and treatment effectiveness.

Required profile

• Bachelor’s degree or certificate from an APTA‑accredited Physical Therapy program.
• Current state licensure as a Physical Therapist.
• Current Basic Life Support (BLS) certification.
• Valid driver’s license with required automobile insurance coverage.
• Ability to meet heavy‑level physical demands, including walking, climbing stairs, and lifting patients.
