Physical Therapist – Home Care (Eastern Cuyahoga)

About the role

Join the Cleveland Clinic Health System as a Home Care Physical Therapist serving patients in Eastern Cuyahoga County. Provide in‑home therapy to improve strength, range of motion, endurance and coordination while building strong patient relationships.

Key responsibilities

• Evaluate and conduct medically prescribed physical therapy treatment programs.
• Perform initial and ongoing assessments and develop plans of care.
• Deliver physical therapy interventions using standard techniques appropriate for the patient’s age and condition.
• Supervise physical therapy assistants, students and other support caregivers.
• Maintain complete, up‑to‑date documentation of patient condition and treatment effectiveness.

Required profile

• Bachelor’s degree or certificate from an APTA‑accredited Physical Therapy program.
• Current state licensure as a Physical Therapist.
• Current Basic Life Support (BLS) certification.
• Valid driver’s license and proof of required automobile insurance coverage.

Required skills

• Physical therapy assessment and treatment techniques.
• Basic Life Support (BLS) skills.
• Ability to document patient care accurately.
