Physical Therapist – Home Care (East of I‑77)

About the role

The Cleveland Clinic Health System is seeking a licensed Physical Therapist to provide home‑based rehabilitation services to patients in the East of I‑77 area. You will work Monday through Friday, 8:00 am‑5:00 pm, with occasional weekend shifts and a holiday weekend.

Key responsibilities

• Evaluate and conduct medically prescribed physical therapy treatment programs in patients' homes.
• Perform initial and ongoing assessments, establish and revise plans of care, and document patient progress.
• Supervise physical therapy assistants, students, and other support caregivers.
• Participate in interdisciplinary assessments and maintain up‑to‑date patient records.

Required profile

• Bachelor’s degree or certificate from an APTA‑accredited Physical Therapy program.
• Current state licensure as a Physical Therapist.
• Current Basic Life Support (BLS) certification.
• Valid driver’s license with required automobile insurance coverage.
• One year of clinical experience in an acute, rehab, or skilled‑nursing setting (preferred).

• Ability to assess, document, and modify treatment plans based on patient response.
