Per Diem On-Call RN – Evening Home Health

About the role

BAYADA at Bayhealth is seeking an experienced Per Diem On-Call Registered Nurse to provide evening home‑health coverage for adult and geriatric patients throughout Kent County, DE. This flexible on‑call position involves in‑home skilled nursing visits, start‑of‑care admissions, and delivering patient‑centered care with support from a seasoned clinical team.

Key responsibilities

• Respond to after‑hours patient needs within the assigned coverage area.
• Provide skilled nursing care including medication administration, wound care, and prescribed treatments.
• Monitor patient conditions, communicate changes to the Clinical or Client Services Manager, and document care in the EMR.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.

Required profile

• Current RN license in the State of Delaware.
• Minimum one year of recent home‑health nursing experience.
• Experience completing OASIS documentation and Start of Care visits.
• Reliable transportation to travel throughout Somerset, Union, Middlesex, and Monmouth Counties.
