Patient Navigator – Support Patients Through Care Coordination

About the role

The Patient Navigator at the University of Miami Health System helps patients overcome barriers to care, coordinates treatment plans, and ensures follow‑up services are delivered smoothly. This role acts as a bridge between patients, providers, and community resources to improve health outcomes.

Key responsibilities

• Conduct patient evaluations to gather information for effective treatment planning.
• Verify and facilitate patient access to insurance and eligible social service programs.
• Provide appointment reminders and serve as a liaison between patients and healthcare providers.
• Identify, address, and resolve real or perceived barriers to care.
• Educate and counsel patients on health‑related and life‑situational issues.
• Maintain up‑to‑date knowledge of community health, welfare, and support resources.
• Handle patient communications, request medical records, and coordinate with nurse navigators to close care gaps.
• Prepare, track, and process medical records and pathology specimens, ensuring completeness and confidentiality.
• Provide administrative support including answering phones, directing calls, copying, faxing, emailing, and mailing records.

Required profile

• High school diploma or equivalent.
• Minimum of 1 year of relevant work experience.
• Ability to handle confidential information with discretion.
• Strong oral and written communication skills.
• Capacity to manage stressful situations with professional composure.
• Effective interpersonal skills and ability to follow instructions.
