Occupational Therapist – Home Health (West of I‑77)

About the role

The Occupational Therapist – Home Health provides in‑home patient care across the west side of the Cleveland area, supporting individuals with orthopedic, neurological, and cardiovascular conditions. The role involves evaluations, start‑of‑care visits, ongoing treatment, discharge planning, and equipment recommendations.

Key responsibilities

• Perform initial and ongoing assessments of patients’ conditions and develop individualized plans of care.
• Deliver therapy interventions using standard occupational therapy techniques, including wheelchair and durable medical equipment assessments.
• Supervise occupational therapy assistants, students, and other support personnel.
• Document patient progress and maintain up‑to‑date records, often completing documentation from home.
• Participate in interdisciplinary care conferences and hospice team meetings as needed.

Required profile

• Bachelor’s degree from an AOTA‑accredited Occupational Therapy program.
• Current Ohio licensure as an Occupational Therapist.
• Current Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting policy requirements.
• Two years of OT experience, preferably with home‑care exposure.

Required skills

• Proficiency in occupational therapy assessment and intervention techniques.
• Ability to conduct wheelchair and equipment evaluations.
• Competence in documenting care using mobile technology.
• Basic Life Support (BLS) skills.
