Nursing Admission Specialist – Home Care

About the role

The Nursing Admission Specialist provides skilled nursing, assessment, and coordination of short‑term intermittent home care for patients discharged from hospitals or outpatient procedures. Working independently in patients' homes across Eastern Cuyahoga, Geauga, Lake, and Ashtabula counties, the RN guides patients through recovery, ensuring safe transitions and high‑quality care.

Key responsibilities

• Perform lab draws (peripheral or central line) and manage wound care, including measurements and documentation.
• Insert and manage IV therapy, administer IV antibiotics, push medications, and handle central lines.
• Educate patients and families on recovery plans, medication administration, and home health equipment.
• Coordinate care with the HC Nursing Team East‑Center for Connected Care and maintain accurate OASIS documentation.

Required profile

• Graduate of an accredited nursing program with current Ohio RN licensure.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting specified coverage.
• Minimum one year of RN experience in acute medical/surgical or home‑care settings.
