Nurse Practitioner PRN

About the role

Arkos Health, a value-based healthcare company serving payors and providers, is hiring a Nurse Practitioner for Cheyenne, WY and surrounding areas. The role covers assessment, diagnosis, treatment, care planning, education, health promotion and care coordination for members with acute and chronic conditions. Full-time and PRN options are available, with a Monday to Friday schedule.

Key responsibilities

• Perform complete, accurate in-home history and physical assessments, including acute, palliative, CHA and home health requests, at residences, facilities or provider offices
• Review member records and interpret diagnostic studies and screenings to support diagnosis and care plans
• Develop, implement, evaluate and adjust treatment plans and make medication recommendations
• Maintain accurate electronic health record documentation, including diagnosis codes, condition status and care plans
• Learn the RAF and HCC models and Quality STAR/HEDIS measures within 30 days of hire
• Rotate telephonic on-call coverage after hours

Required profile

• Current and valid Wyoming RN license, plus Bachelor's degree in Nursing and Wyoming Advanced Practice Nursing license (Master's preferred)
• Board certification for Nurse Practitioners and unrestricted prescribing/dispensing privileges with DEA licensure
• At least 1 year of ambulatory or community care experience, or equivalent clinical experience
• Preferred: experience with adults and older adults, and population health management
• Driving required, with current license and proof of insurance
