Nurse Practitioner – Onsite Care for Senior Living Residents

About the role

Ultra Health Providers is seeking a Nurse Practitioner to deliver high‑presence, quality medical care within skilled nursing and senior living communities. You will act as the primary clinical presence, providing proactive, patient‑centered care and responding to urgent medical needs to ensure continuity, dignity, and optimal health outcomes for residents.

Key responsibilities

• Provide primary, urgent, and preventive medical services to residents in skilled nursing and assisted‑living settings.
• Independently evaluate, diagnose, and treat acute and chronic conditions.
• Conduct admissions, follow‑up assessments, physical exams, medication reviews, and appropriate screenings.
• Monitor resident condition changes and adjust treatment plans as needed.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate diagnoses, treatment plans, and care updates clearly and compassionately to residents and families.

Required profile

• Licensed Nurse Practitioner with experience in senior or skilled‑nursing environments.
• Comfortable working independently while integrating into a collaborative interdisciplinary team.
• Strong clinical autonomy, patient‑centered decision‑making, and ability to manage complex medical conditions.
• Excellent communication skills for interacting with residents, families, and healthcare staff.
