Nurse Practitioner - Care at Home

About the role

Optum Home & Community Care, part of the Optum family, is hiring a Nurse Practitioner for its Care at Home program in Des Moines, Iowa. The program coordinates clinical care for high-risk health plan beneficiaries in their own homes, working with an interdisciplinary team. The position involves daily travel to patient homes within the assigned area, with no nights, weekends or holidays.

Key responsibilities

• Conduct comprehensive in-home assessments, including past medical history review, medication reconciliation, vital signs and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Manage members through acute situations or exacerbations of chronic conditions, including testing, diagnostics and interventions
• Manage members during short-term sub-acute rehabilitation episodes
• Ensure accurate and complete documentation, including ICD-10 conditions
• Collaborate with the interdisciplinary care team and conduct advanced illness and care planning conversations
• Counsel and educate patients and caregivers

Required profile

• Active ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care
• Active, unrestricted state licensure (or ability to obtain it by start date) without disciplinary actions or practice limitations
• Active, unrestricted driver's license and reliable transportation
• Current, active DEA licensure or prescriptive authority, or ability to obtain it after hire (unless prohibited in the state)
• Ability to obtain a collaborative practice agreement where applicable
• Ability to navigate varied environments and to sit, stand or kneel as needed

Preferred qualifications

• 1+ years of clinical experience as an APC (family, geriatric or home health preferred)
• Experience in geriatric medicine, long-term care, senior living or home care
• Understanding of geriatrics, chronic illness, advanced illness and end-of-life discussions
• Proficiency with electronic medical records and technology

• Salary range of $109,500 - $164,000, plus a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (subject to eligibility)
