Nurse Navigator 1 – Primary Care Team-Based Care

About the role

The Nurse Navigator 1 serves as the initial point of contact for patients entering the University of Miami health system, ensuring timely scheduling and coordinated care throughout the primary‑care continuum.

Key responsibilities

• Triages new patient appointments, reviews outside medical records, and schedules initial visits.
• Assesses barriers to care, refers patients to support services, and provides education on treatment plans.
• Coordinates complex referrals, diagnostic testing, and follow‑up across specialties, and facilitates transitions after hospital or emergency‑department visits.
• Mentors and precepts new graduates, students, and visiting professionals.

Required profile

• Bachelor’s degree in a relevant field.
• Minimum 2 years of relevant nursing experience.
• Valid State of Florida RN license.
• Commitment to quality patient care and continuous learning.

Required skills

• Proficiency with office software and relevant computer applications.
• Strong written and verbal communication for interdisciplinary collaboration.
