About the role

Join the Cleveland Clinic Home Care team as a Licensed Practical Nurse providing hospice and home health services. You will support patients in the Akron/Canton region and surrounding areas, delivering skilled nursing care in the comfort of their homes.

Key responsibilities

Visit and provide care to approximately six patients each day in their homes.
Ensure continuity of care and coordinate discharge planning.
Collect and document patient data and report to the supervising RN.
Administer medications and monitor effectiveness and adverse reactions.
Perform dressing changes, laboratory tests, and other direct patient care tasks.

Required profile

Graduate from an accredited Practical or Vocational Nursing Program.
Current Ohio LPN license.
At least one year of clinical experience in an acute setting.
Basic Life Support (BLS) certification.
Valid Ohio driver’s license and automobile insurance.

Required skills

Infusion experience.
Hospice experience.
Medication administration within scope of practice.
