LPN Home Care – Cuyahoga County (West)

About the role

The Licensed Practical Nurse will provide skilled home‑care services to patients in West Cuyahoga and Lorain County, supporting their recovery and independence after hospital stays or procedures.

Key responsibilities

• Visit approximately six patients daily in their homes, delivering nursing care, medication administration, dressing changes, and basic lab tests.
• Collect and document patient data, report findings to the supervising RN, and coordinate discharge planning.
• Educate patients and families about home‑care benefits, safety measures, and care plans.
• Observe patients for safety risks, implement appropriate precautions, and communicate any concerns to the health‑care team.

Required profile

• Graduate of an accredited practical or vocational nursing program.
• Current Ohio LPN license with at least one year of acute‑care nursing experience.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and proof of automobile insurance meeting required coverage.

Required skills

• Medication administration within scope of practice.
• Knowledge of JCAHO and Medicare standards.
• Infusion experience (preferred).
• M‑IV certification (preferred).
