Licensed Practical Nurse – Homecare (East Cuyahoga)

About the role

The Licensed Practical Nurse will provide skilled homecare to patients in the East Cuyahoga area, delivering nursing, therapy coordination and education in patients' homes.

Key responsibilities

• Visit approximately six patients per day, providing direct nursing care, medication administration, dressing changes and basic lab tests.
• Collect and document objective and subjective patient data, report findings to the supervising RN and contribute to care plans.
• Coordinate discharge planning, ensure continuity of care and educate patients and families about homecare benefits.
• Identify safety risks, implement appropriate safety measures, and report any unusual conditions to the health‑care team.

Required profile

• Graduate of an accredited practical or vocational nursing program.
• Current Ohio LPN license.
• At least one year of clinical experience in an acute care setting.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license with automobile insurance meeting $100,000/$300,000 coverage.

Required skills

• Knowledge of JCAHO and Medicare standards and regulations.
• M‑IV certification and infusion therapy skills (preferred).
• Ability to drive to patients’ homes, climb stairs and safely transport equipment.
