Licensed Practical Nurse - Home Health

About the role

UChicago Medicine AdventHealth is hiring a full-time Licensed Practical Nurse (LPN) for its home health program covering Oak Lawn and Chicago Ridge. The schedule is a day shift Monday to Friday, 8am to 5pm, with rotating call and weekend coverage. Mileage reimbursement of $0.76 per mile is provided, and flexible scheduling is also available for part-time and PRN positions.

Key responsibilities

• Provide direct patient care under the direction of the RN, including medication administration, wound dressing changes, ostomy care and patient education.
• Assess patients during clinical check-ins, document medical information and vital signs, and look for signs of abuse or neglect.
• Perform treatments and follow-up assessments ordered by the provider, and respond to clinical emergencies and triage situations.
• Perform diagnostic and occupational medicine testing such as EKG, spirometry, audiometry, titmus and pulse oximetry, and collect forensic specimens according to guidelines.
• Receive and verify physician orders, ensuring they are co-signed by an RN, and report observations to the attending physician and RN.
• Maintain clinical records within established standards and time frames, and communicate with the home health care team.
• Participate in staff development, the review process and the agency performance improvement program.

Required profile

• Licensed Practical Nurse (LPN).
• High school graduate or equivalent, with a technical or vocational school background in LPN.
• 1+ year of medical/surgical experience preferred.
• Basic Life Support (CPR) certification and a driver's license; auto insurance preferred.
• Background check meeting organizational guidelines.

Required skills

• Practical nursing knowledge, venipuncture skills and basic computer skills including MS Office.
• Point of care documentation software knowledge preferred.
• Proficiency in English; additional languages preferred.
