Licensed Medical Social Worker – Home Health (Part‑time)

About the role

BAYADA Home Health Care is seeking an energetic Licensed Medical Social Worker (LMSW) for a part‑time, per‑diem position providing home health visits across designated territories. You will join a multidisciplinary team delivering skilled nursing and rehabilitative care to help clients remain safely at home.

Key responsibilities

• Conduct home visits to clients in assigned geographic areas.
• Perform clinical social evaluations, develop intervention plans, and document findings using a tablet.
• Identify and coordinate community resource referrals to meet clients' and families' practical needs.
• Advocate for clients and families, navigating them through public and private support systems.

Required profile

• Licensed at the Master (LMSW) or Independent (LISW) level by the state board.
• Master’s degree in Social Work with at least 2 years of community‑based experience.
• Strong written and verbal communication skills in English.
• Ability to work independently, manage time effectively, and travel to client sites.

Required skills

• Proficiency with electronic medical records (EMR) systems.
