Integrated High Needs Care Coordinator

About the role

Southwest Behavioral Health Services is seeking a compassionate Integrated High Needs Care Coordinator to join its Lake Havasu team. The role involves providing case management for clients with complex medical and co‑occurring substance‑use disorders, aiming to improve wellness and reduce emergency visits.

Key responsibilities

• Facilitate CFT meetings and develop individualized service plans (ISPs) for assigned families.
• Provide education, support, and resource linkage to youth, families, and team members.
• Maintain accurate documentation, progress notes, and safety/support plans.
• Participate in on‑call rotation and ensure confidentiality per HIPAA standards.

Required profile

• High school diploma or GED with at least five years of full‑time integrated/behavioral health experience (Bachelor’s preferred).
• Valid Arizona driver’s license, fingerprint clearance, and ability to transport clients as needed.
• Strong knowledge of DSM, community resources, psychotropic medications, and crisis intervention.

Required skills

• Excellent communication and organizational abilities.
• Ability to coordinate interdisciplinary teams and advocate for clients.
