Inpatient Care Manager – MSW

About the role

The Inpatient Care Manager – MSW coordinates comprehensive care for a designated patient population across the continuum of care. This clinician‑care manager‑educator role collaborates with interdisciplinary teams to ensure safe, timely discharge planning and high‑quality, cost‑effective outcomes.

Key responsibilities

• Conduct comprehensive assessments at patient entry to determine length of stay, discharge needs and resources.
• Develop and implement individualized care and transition plans with the multidisciplinary team.
• Coordinate safe and timely discharge planning, including post‑acute authorizations, transportation, home‑care and facility placements.
• Monitor patient progress through daily reviews and multidisciplinary rounds, identifying and resolving barriers to discharge.
• Advocate for patients and families, addressing clinical, educational and psychosocial needs.
• Maintain knowledge of Medicare, Medicaid and payer reimbursement methodologies.

Required profile

• Master’s Degree in Social Work.
• Less than 1 year experience in an MSW internship program.
• Ability to interact with diverse backgrounds and set priorities for care coordination.
• Proven leadership skills and familiarity with computer applications and analytical tools.

Required skills

• Experience with EPIC electronic health record system.
• Knowledge of case management certifications (ACM or CCM) within three years.
