Inpatient Care Manager - BSN

About the role

The Inpatient Care Manager – BSN leverages nursing expertise to coordinate comprehensive care for a designated patient population across the continuum of care. This clinician‑care manager‑educator role ensures safe, timely discharge planning and supports performance improvement initiatives.

Key responsibilities

• Conduct comprehensive assessments at patient entry, develop individualized care and transition plans, and determine anticipated length of stay.
• Coordinate safe and timely discharge planning, including post‑acute authorizations, transportation, home‑care, and facility placements.
• Monitor patient progress through daily reviews and multidisciplinary rounds, identifying and resolving barriers to discharge.
• Advocate for patients and families, addressing clinical, educational, and psychosocial needs throughout the care continuum.

Required profile

• Bachelor’s Degree in Nursing with at least 3 years of experience in care coordination, disease management, home health, hospital, or physician office settings.
• One year of experience using an electronic medical record (EMR) system.
• RN licensure in Pennsylvania (or New Jersey upon hire) and ability to obtain required certifications.
• Proven leadership skills and ability to work with diverse populations.

Required skills

• Electronic Medical Record (EMR) systems, specifically EPIC.
• Accredited Case Manager (ACM) certification or Certified Case Manager (CCM) certification within three years.
• Knowledge of Medicare, Medicaid, and payer reimbursement methodologies.
• Proficiency with computer applications and analytical tools.
