About the role
AdventHealth is seeking a part‑time Hospice Medical Social Worker to join its interdisciplinary team in Castle Rock. The role focuses on assessing and supporting patients’ mental, psychosocial, cultural, and spiritual needs while linking them to community resources.
Key responsibilities
- Plan, implement, and organize social work resources and evaluate treatment for patients with mental health issues.
- Assess patients’ physical, psychosocial, cultural and spiritual needs through observation, interview, and review of medical records.
- Develop and implement patient care plans, monitor outcomes, and collaborate with the interdisciplinary team.
- Demonstrate competency in a variety of therapeutic and diagnostic interventions.
Required profile
- Master of Social Work (MSW) degree.
- Current Basic Life Support (BLS) certification from the American Heart Association.
