Home Infusion Clinical Appeals Pharmacist

About the role

The Home Infusion Clinical Appeals Pharmacist provides comprehensive pharmaceutical care for intravenous home‑care patients, supports referrals, and oversees the Patient Advocate team.

Key responsibilities

• Compound sterile medications and infusions for home, outpatient clinics, skilled nursing facilities, and physician offices.
• Initiate and evaluate infusion therapies, complete required fiscal and legal documentation, and coordinate deliveries with nursing and support services.
• Act as pharmacist in charge in the absence of the PIC, make on‑duty decisions, and handle on‑call emergency duties.
• Participate in inventory control, ordering, warehouse organization, hazardous waste management, and equipment maintenance.

Required profile

• Bachelor of Science in Pharmacy (required); PharmD preferred.
• Completion of a PGY1 or PGY2 Pharmacy Practice Residency.
• License to practice pharmacy in the state of practice.
• Preferred: 2 years experience as a pharmacist, especially in specialty pharmacy.

Required skills

• Computer proficiency for data entry, Windows‑style applications, and pharmacy‑specific software.
• Effective verbal and written communication with patients, caregivers, and healthcare teams.
• Ability to compound sterile medications and manage home infusion therapies.
• Knowledge of federal, state, and local regulations governing pharmacy practice.
