Home Health Speech Language Pathologist (Per Diem)

About the role

BAYADA Home Health Care seeks an experienced Speech Language Pathologist to provide per‑diem home health services to adult and geriatric patients in Lexington, Denton and parts of Winston‑Salem, NC.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and treat speech, language, communication, or swallowing difficulties.
• Develop and continually revise treatment plans based on evaluations, physician orders, and client goals.
• Educate patients, families, and caregivers on rehabilitative care to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a tablet‑based electronic system.

Required profile

• Current North Carolina license as a Speech Language Pathologist.
• Certificate of clinical competence in speech from the American Speech and Hearing Association.
• Experience working with adult and geriatric populations.
• Solid computer skills; prior experience with electronic medical records preferred.
