Home Health RN Case Manager

About the role

James River Home Health is seeking a compassionate Home Health RN Case Manager to manage a caseload of patients in their homes, collaborating with an interdisciplinary team to ensure high‑quality care.

Key responsibilities

• Provide skilled nursing care to homebound patients according to the Plan of Care and develop individualized treatment plans.
• Coordinate multidisciplinary services, handling admissions, transfers, discharges and ensuring continuity of care.
• Educate patients and families on health maintenance, complete timely electronic documentation in Homecare Homebase, and offer emotional support throughout the care journey.

Required profile

• Registered Nurse with a current Virginia license.
• At least one year of home health experience preferred.
• Valid driver’s license and reliable vehicle for work hours.

Required skills

• Proficiency with OASIS assessments.
• Experience using Homecare Homebase (HCHB) system.
