Occupational Therapist - Home Health Visits

About the role

BAYADA Home Health Care is hiring an experienced Occupational Therapist for a full-time position making home health visits for its Raleigh Visits office, which serves adult and geriatric patients in Johnston County on a per-visit basis. You will be part of a multi-disciplinary team delivering rehabilitative care and skilled nursing so clients can stay at home.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate patients' functional status, including muscle function, endurance, visual coordination, communication skills and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects of performance and develop individualized intervention plans.
• Evaluate the home environment for hazards or barriers to independent living and identify equipment needs and environmental adaptations.
• Continually revise the occupational therapy care plan and take part in the multidisciplinary care plan with nursing.
• Educate patients, family members and other patient representatives on rehabilitative care and activities that support health, safety and independence.
• Document observations, interventions and evaluations using a touch-pad tablet.

Required profile

• A current North Carolina Occupational Therapist license.
• At least one year of recent work experience as an Occupational Therapist.
• Diploma or transcript showing graduation from an occupational therapy program accredited by the AMA Committee on Allied Health Education and Accreditation and the AOTA.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time effectively, with strong interpersonal, organizational and problem-solving skills.

Required skills

• Solid computer skills.
• Prior experience with electronic medical records (EMR) preferred.
