Home Health Registered Nurse Clinical Manager

About the role

AdvisaCare is looking for a compassionate leader to act as its Home Health Registered Nurse Clinical Manager in Flint. As the heartbeat of the clinical operations, the role blends leadership, clinical expertise and relationship-building, supporting clinicians and helping patients and families receive safe, high-quality, personalized care.

Key responsibilities

• Support and mentor a team of nurses and caregivers
• Ensure patients receive safe, high-quality, personalized care
• Oversee clinical documentation, care plans and regulatory compliance
• Collaborate with physicians, referral partners and families
• Lead case conferences and guide clinical decision-making
• Help onboard, train and inspire new team members
• Monitor quality measures, patient satisfaction and outcomes

Required profile

• Active Michigan RN license
• 2-3 years of clinical experience in home health, hospice or a related field preferred
• Previous leadership or supervisory experience
• Bachelor of Science in Nursing (BSN) is a plus
• Ability to work in the field as needed; eligible to work in the United States

Required skills

• Solid understanding of Medicare home health OASIS is a plus
• Comfort with EMR systems and clinical workflows
• Experience with Kinnser is a plus
