About the role

AdvisaCare is a home health and hospice organization offering personalized in-home care across Medicare home health, therapy, respiratory care and hospice. Through its Rebound Physical and Occupational Therapy program, the role centers on helping people regain function, confidence and quality of life after a wide range of conditions. As a Home Health Physical Therapist, you will guide each patient's recovery through thoughtful, goal-driven care.

Required profile

Physical Therapy degree
Licensed Physical Therapist in the state of Michigan
Eligible to work in the United States
Current working knowledge of Medicare Conditions of Participation preferred
Minimum 1 year of home care experience highly preferred
