About the role

We are seeking a skilled Home Health Field Registered Nurse (RN) to deliver high‑quality nursing care to patients in their homes across the Dallas‑Fort Worth Metroplex. This role offers autonomy, supportive leadership, and the chance to make a measurable impact on patients' lives.

Key responsibilities

• Perform in‑home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Provide wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in line with regulatory requirements.
• Participate in on‑call rotation as needed.
• Build trusting relationships with patients and families.

Required profile

• Current RN license in the state of Texas.
• At least 1 year of nursing experience (home health preferred but not required).
• Strong clinical assessment and critical‑thinking abilities.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting systems.
• Compassionate, professional, and patient‑focused attitude.
