Home Health Case Manager

About the role

The Home Health Case Manager provides skilled nursing, social services, and therapy to patients recovering at home after hospital stays or procedures. Working independently in the community, the RN delivers short‑term intermittent acute care, coordinates care, and ensures high‑quality patient outcomes.

Key responsibilities

• Perform assessments, lab draws (peripheral or central), wound care, and IV therapy in patients' homes.
• Educate patients and families throughout the short‑term recovery process and manage patient assignments autonomously.
• Travel to assigned areas in Eastern Cuyahoga County, covering cities such as East Cleveland, Cleveland Heights, South Euclid, and Lyndhurst.

Required profile

• Graduate of an accredited nursing program with current Ohio RN licensure.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and required automobile insurance coverage.
• Minimum one year of RN experience in an acute medical/surgical or home‑care setting.

Required skills

• Peripheral and central line blood draws.
• Comprehensive wound care, including measurements and wound vac use.
• IV insertion, administration of IV antibiotics, IV push medications, and central line management.
