About the role

Join Cleveland Clinic Health System as a Home Health Aide on the Hospice team, providing compassionate care to patients in their homes, nursing facilities, or assisted‑living centers across several counties in southern Ohio.

Key responsibilities

Perform basic patient care activities, including personal hygiene, activities of daily living, and safe transfers.
Assist with physician‑ordered procedures under supervision of an RN, PT, OT, or ST.
Document patient conditions, report abnormal findings, and maintain up‑to‑date records.
Keep the patient’s environment clean, orderly, and safe.

Required profile

High School Diploma or GED.
State Tested Nurse Aide (STNA) certification registered with the Ohio Nurse’s Aide Registry.
Valid Ohio driver’s license with required automobile insurance coverage.
Basic Life Support (BLS) certification (to be obtained during orientation).

Required skills

Ability to ambulate, climb stairs, and lift or move patients as needed.
Proficiency in using personal protective equipment and following standard precautions.
