Home Health Admissions Nurse – Cuyahoga & Summit County

About the role

Cleveland Clinic Home Care delivers skilled nursing, therapy and social services to patients recovering at home after hospital stays or procedures. As an Admissions RN you will guide patients through a short‑term intermittent recovery process, providing autonomous, in‑home assessments and care coordination across Cuyahoga and Summit counties.

Key responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans for patients in their homes.
• Conduct lab draws (peripheral and central line), wound care, IV therapy, and medication administration.
• Educate patients and families on post‑acute care, medication management, and follow‑up instructions.
• Coordinate with the multidisciplinary team to ensure seamless transitions from hospital to home.

Required profile

• Graduate of an accredited nursing program with current Ohio RN licensure.
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance meeting specified coverage.
• Minimum one year of RN experience in acute Medical/Surgical or Home Care settings.
• Knowledge of Home Care and OASIS data set.

Required skills

• Lab draws – peripheral and central line.
• Wound care – all types, measurements, documentation, wound vac.
• IV insertion and therapy – administration of antibiotics, IV push, central line experience.
• Diabetes management – glucometry.
• Urinary catheterization, enteral feeding, oxygen therapy.
• Administration of injectable medication, Left Ventricular Assist Device.
