Full-Time Licensed Practical Nurse (LPN)

About the role

Pavilion, a long-term care facility in Baxley, is hiring a full-time Licensed Practical Nurse (LPN) who works under the direct supervision of the Nurse Manager and/or Charge Nurse. The LPN assists in the daily care of patients by assessing their needs and helping implement the treatment plan. The position offers a sign-on bonus of $3,000 and shifts available 7am to 7pm.

Key responsibilities

• Accompany providers with patient rounding, review diagnostics and carry out doctors' orders
• Intervene and de-escalate patient behavior using CPI interventions and assist with milieu management
• Communicate patient updates to families and nursing home staff, and take part in treatment team meetings and care plan updates
• Ensure infection control precautions are followed, PPE is available and set up at each resident's door, and document rounds on the observation record
• Chart resident information, prepare weekly and monthly summaries, and take call rotations
• Count controlled substances at the start and end of shift, monitor medications and equipment for expiration dates, and change medication cards weekly
• Clean and stock medication carts, maintain refrigerator temperature logs, and tag non-working equipment with a work order
• Complete unit safety checks, order supplies, enter patient charges, and assist with 24-hour chart checks
• Act as a preceptor for nursing students and orient new hires, and contribute to QAPI and problem-solving efforts
• Keep the workstation clean and help with housekeeping duties

Required profile

• Graduate of an accredited school of Licensed Practical Nursing
• Current state of Georgia LPN license, maintained with 20 hours of continuing education every two years
• Current Basic Cardiac Life Support certification
• Prior experience in a long-term care facility preferred but not required
