Field Social Worker

About the role

UnitedHealth Group is looking for a compassionate and experienced Social Worker to support high-risk members in the Phoenix Metro (West Valley) area. You will address social determinants of health, behavioral health needs and barriers to care, working as part of an interdisciplinary care team to improve outcomes and reduce avoidable utilization.

Key responsibilities

• Conduct psychosocial, SDOH, caregiver and behavioral health assessments
• Identify barriers to care and connect members to community resources and support services
• Develop and implement individualized care plans with the interdisciplinary team
• Coordinate referrals for housing, food, transportation, utilities, caregiver support and other community services
• Help members with Medicaid, Medicare Savings Programs, Extra Help/LIS and D-SNP eligibility and recertification
• Support care transitions and post-discharge planning to reduce readmissions
• Facilitate advance care planning and goals-of-care discussions
• Provide crisis intervention, elder abuse and neglect screening, and mandated reporting as required
• Build relationships with community organizations and follow up on referrals

Required profile

• Active, unrestricted LCSW or LISW license
• 2+ years of medical, care management or community social work experience
• Valid driver's license and reliable transportation

Preferred qualifications

• Advance Care Planning (ACP) facilitator certification
• Geriatric, hospice or palliative care experience
• Medicare and Medicaid benefits expertise
• Bilingual English/Spanish

• Salary of $60,200 - $107,400 annually for full-time employment, depending on factors such as experience and certifications
