Field Care Coordinator - Sevier County

About the role

UnitedHealthcare is hiring a Field Care Coordinator for Sevier County, Tennessee. The role applies clinical knowledge to coordinate care for medically complex members who need intensive medical and psychosocial support, working directly with members to ensure access to services and providers. It is a field-based position; candidates living in or near Sevier County can work remotely.

Key responsibilities

• Apply clinical knowledge to manage Medicare/Medicaid cases efficiently and use evidence-based practices in care coordination
• Deliver individualized self-management support and patient education, and refer members to community resources
• Conduct telephone and face-to-face assessments per Health Plan policy, and develop and maintain individual care plans
• Act as member advocate, resolve barriers to care, and coordinate services in line with Health Plan policy
• Monitor hospital encounters and intervene to reduce their frequency
• Refer clinical decisions outside the coordinator's authority to a manager for review
• Take part in quality improvement, use Health Plan software to log service requests and keep time-sensitive documentation
• Travel up to 75% within the assigned territory to meet members and providers

Required profile

• Bachelor's degree in Social Work, Nursing or another health care profession including LPN
• 2+ years of clinical case management experience
• Ability to travel up to 75% within the territory
• Resides in or near Sevier County with reliable transportation and a valid US driver's license
• Preferred: LCSW, LBSW, CCM certification, long-term care experience, Medicare/Medicaid and medical terminology knowledge
