Field-Based HSS Clinical Coordinator RN

About the role

UnitedHealthcare is hiring a Field-Based HSS Clinical Coordinator RN in Harris County, TX. The role involves assessing, planning and implementing person-centered strategies, connecting members with resources and coordinating care across the continuum. Candidates living in Harris County and nearby commuting areas may work remotely.

Key responsibilities

• Assess, plan and implement person-centered strategies
• Connect members with financial, psychosocial, community and state resources
• Coordinate care across the continuum and act as a key point of contact
• Advocate for members and families so their needs and preferences are heard and supported
• Collaborate with providers, members, caregivers and community partners to improve health outcomes
• Travel 50% of the time within Harris County and surrounding areas

Required profile

• Current unrestricted Texas RN license
• 2+ years of experience in a community health or healthcare setting
• Reliable transportation, a valid driver's license and ability to travel within the assigned territory
• High-speed internet at residence

Required skills

• Intermediate proficiency with Microsoft Office applications
• Preferred: experience with Medicaid, LTSS or long-term care services
• Preferred: knowledge of HCBS and integrated care principles, and of federal and state home and community-based settings regulations
• Preferred: bilingual English and Spanish

What we offer

• Hourly pay of $29.00 to $52.00 for full-time employment, depending on factors such as local labor market, education, experience and certifications
