Community Liaison – House Calls (Nassau County, NY)

About the role

Ennoble Care is looking for a full‑time Housecall Community Liaison to expand its home‑based primary, palliative, and hospice services across Nassau County, Long Island. You will act as the public face of the company, building relationships with healthcare providers and community organizations to generate patient referrals.

Key responsibilities

• Identify and target potential referral sources such as doctors’ offices, hospitals, assisted‑living and skilled‑nursing facilities.
• Present Ennoble Care’s credentials and service offerings to prospective partners.
• Analyze market data, company strengths/weaknesses, and develop a strategic marketing plan with clear timelines.
• Implement the marketing plan, monitor progress, and adjust tactics as needed.
• Attend internal meetings, support office updates, and provide community outreach and education.

Required profile

• Minimum three years of experience in marketing, nursing, or social work interacting with health agencies or professionals.
• Highly organized, detail‑oriented, and able to communicate clearly both orally and in writing.
• Valid driver’s license, insured automobile in good working order.

• Strong analytical abilities for market and data assessment.
• Effective presentation and relationship‑building skills.

What we offer

• Base salary ranging from $50,000 to $95,000 per year, plus monthly, quarterly, and annual bonuses.
