Community Health Worker

About the role

The Community Health Worker (CHW) will serve diverse, underserved populations by linking them to health and social services. Working primarily in the community, the CHW collaborates with medical providers, primary‑care teams, and external agencies to improve patient outcomes.

Key responsibilities

• Establish trusting relationships with patients and families, providing support, education, and counseling.
• Visit patients at home to assess living conditions, assist with care plans, and coordinate services.
• Advocate for patients by acting as a liaison between families and community agencies such as schools, hospitals, and support groups.
• Document activities, service plans, and outcomes in compliance with policies and procedures.

Required profile

• High school diploma/GED and a valid driver’s license.
• Community Health Worker certification through the Board of Nursing.
• Strong information‑gathering, interviewing, and time‑management abilities.
• Preferred: one year of medical office experience, two years of community service, bilingual English/Spanish.

Required skills

• CHW certification.
• Experience with Epic and other Electronic Medical Record (EMR) systems.
• Medical Assistant training or certification.
• Bilingual proficiency in English and Spanish.
