About the role
The Community Health Worker Apprentice supports patients, the healthcare system, and community organizations to improve health outcomes for underserved populations. Working under a seasoned CHW preceptor, you will gain hands‑on experience while attending hybrid coursework.
Key responsibilities
- Build and maintain community partnerships, promote health equity, and connect patients with essential resources.
- Assist patients in navigating the healthcare system, provide informal counseling, and support outreach education efforts.
Required profile
- High school diploma or equivalent and residence in Northeast Ohio.
- Strong interpersonal, communication, and time‑management skills, with ability to develop trust with patients and staff.
Required skills
- Experience or familiarity with Epic electronic health record system.
- Bilingual Spanish preferred.
