Clinical Pharmacist – Hematology/Oncology

About the role

The Clinical Pharmacist will serve as the primary pharmacist for the inpatient Hematology/Oncology service at a 256‑bed hospital, supporting a high‑acuity patient population and a robust bone marrow and stem cell transplant program.

Key responsibilities

• Participate in multidisciplinary rounds and collaborate with providers on patient care plans.
• Review, verify, and optimize chemotherapy and supportive care medication orders in Epic.
• Perform clinical medication monitoring, therapeutic interventions, and ensure medication safety and accuracy.
• Provide drug information, monitor patients for efficacy, toxicity, and adherence to treatment protocols.
• Support oncology and transplant‑related medication management and assist with pharmacy staffing as needed.

Required profile

• PharmD or Bachelor’s of Science in Pharmacy from an accredited college.
• Current state licensure as a Registered Pharmacist (RPh) or eligibility within 90 days of hire.
• Basic Life Support (BLS) certification and, if required, immunization administration certification.
• Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification, or ability to obtain within one year of hire.

Required skills

• Proficiency with Epic electronic health record system.
• Experience in chemotherapy order verification and supportive care medication management.
• Knowledge of medication safety initiatives and regulatory compliance.
• Ability to educate and train students, residents, and other caregivers.
