Clinical Pharmacist – Ambulatory Endocrinology & Rheumatology

About the role

The Clinical Pharmacist will manage medication therapy for patients in ambulatory endocrinology and rheumatology, ensuring safe, effective, and cost‑efficient treatment. The role includes education of residents, staff, and patients, as well as contributing pharmacy expertise to order sets and formularies.

Key responsibilities

• Provide pharmacokinetic consultation and use laboratory data to guide drug selection and dosing.
• Evaluate patient response to therapy, document interventions, and conduct outcome studies.
• Develop pathways, best practices, and guidelines for the assigned patient population.
• Educate staff, students, patients, and other health‑care professionals; mentor residents and new staff.
• Participate in medication reconciliation, monitoring, and patient education across transitions of care.

Required profile

• Bachelor of Science in Pharmacy (required); PharmD and PGY2 residency preferred.
• Current state pharmacy license.
• Effective verbal and written communication skills.
• Strong leadership and teaching abilities.
