Clinical Care Coordinator – Home Care (CNA/HCA)

About the role

Journey Nursing Services is seeking a Clinical Care Coordinator (CNA/HCA) to lead a caseload of home‑care clients while supporting and mentoring field caregivers. This full‑time position blends hands‑on client care, case management, quality assurance, and leadership.

Key responsibilities

• Manage an assigned caseload, conduct Start of Care assessments, supervisory visits, and develop individualized Plans of Care.
• Orient, train, mentor, and evaluate caregivers, ensuring compliance with each client’s care plan.
• Maintain documentation, coordinate physician orders, and participate in quality‑improvement initiatives.
• Collaborate with scheduling to secure staffing, anticipate coverage needs, and support on‑call rotation.

Required profile

• Current Washington State CNA or Home Care Aide certification and CPR/BLS certification.
• Minimum two years of caregiving experience with strong leadership, communication, and problem‑solving abilities.
• Valid Washington driver’s license, reliable transportation, and willingness to travel throughout the service area.
