Clinical Care Coordinator (CNA/HCA)

About the role

Journey Nursing Services is seeking a Clinical Care Coordinator (CNA/HCA) to lead a caseload of home‑care clients while supporting caregivers in the field. This full‑time, 40‑hour per week position blends hands‑on client care, case management, quality assurance, and team leadership.

Key responsibilities

• Manage an assigned caseload, conduct Start of Care assessments, supervisory visits, and develop individualized Plans of Care.
• Orient, train, mentor, and evaluate caregivers; ensure compliance with client care plans and agency policies.
• Maintain documentation, coordinate physician orders, and participate in quality‑improvement initiatives.
• Collaborate with scheduling to secure appropriate staffing and support on‑call caregiver rotation.

Required profile

• Current Washington State CNA or HCA certification and CPR/BLS certification.
• Minimum two years of caregiving experience with strong leadership, organizational and communication skills.
• Valid Washington driver’s license, reliable transportation, and willingness to travel throughout the service area.
