About the role

The Case Management Social Worker at Cleveland Clinic Fairview Hospital provides comprehensive psychosocial assessments, counseling, and linkage to community resources. This PRN position supports patients with complex discharge planning and crisis intervention needs, ensuring safe transitions of care.

Key responsibilities

• Coordinate care and services for selected patients, addressing complex psychosocial and discharge planning challenges.
• Conduct detailed psychosocial assessments and provide supportive counseling.
• Offer crisis interventions to patients and families and facilitate transitions for high‑risk patients.
• Manage all aspects of discharge planning, including education on post‑acute options and financial concerns.
• Advocate for patients in cases of abuse, domestic violence, and other safety issues.
• Serve as a resource, mentor, and preceptor for less experienced caregivers and represent the department on committees.

Required profile

• Master’s degree in Social Work (MSW) or Master of Science in Social Administration (MSSA).
• Current state licensure as a Social Worker (LSW, LISW, or LISW‑S).
• Basic Life Support (BLS) certification.
• Valid Ohio driver’s license and automobile insurance with required coverage.
• Commitment to meet mandatory and continuing education requirements.
