Care Manager (CNA) - All Shifts

About the role

Monarch/Brandywine Senior Living is hiring a Care Manager (CNA) in Litchfield, reporting to the Health & Wellness Director. The position is full-time and/or part-time, non-exempt and paid hourly, covering all three shifts: 1st shift part-time/per diem, 2nd shift full-time/part-time/per diem, and 3rd shift part-time weekends and per diem. The salary range is $21.00-$22.00 per hour.

Key responsibilities

• Assist residents in all aspects of their daily life according to their care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization and activities.
• Provide care such as bathing, dressing, toileting and feeding as outlined in each resident's care plan.
• Maintain the safety, comfort, dignity and confidentiality of all residents.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe and fed during their shifts.
• Document all services provided to assigned residents during the shift.
• Report any change of condition in a resident immediately to the supervisor.

Required profile

• Certified Nursing Assistant or Certified Home Health Aid.
• Knowledge and experience working with the elderly population, preferred with dementia experience.
• Able to stand, walk, sit, use fine and gross motor skills, balance, stoop, kneel and crouch, and to lift or carry up to 50 lbs with proper body mechanics.
