Care Coordinator – Hepatology

About the role

The Hepatology Care Coordinator works within Cleveland Clinic Health System’s Florida Weston Hospital to provide longitudinal disease management and care coordination for patients with chronic liver conditions. The role focuses on improving treatment outcomes and reducing unnecessary Emergency Department visits.

Key responsibilities

• Collaborate with multidisciplinary teams to develop patient goals, care plans, and interventions.
• Identify high‑risk patients, conduct targeted outreach, and manage their care using registries and patient lists.
• Perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social and end‑of‑life needs.
• Educate patients and families, monitor compliance with care plans, and adjust plans based on progress.
• Facilitate transitions of care, address barriers, and connect patients to community resources.

Required profile

• Graduate from an accredited nursing program and hold current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience, preferably in transplant, GI, ICU, or endoscopy settings.

Required skills

• Proficiency with assessment tools and risk‑assessment instruments.
• Experience using electronic registries, patient lists, and care‑team coordination platforms.
• Ability to perform comprehensive clinical assessments and documentation.
