Care Coordinator - Hepatology

About the role

The Care Coordinator – Hepatology works within the Transplant Center at Florida Weston Hospital to provide longitudinal coordination and disease management for patients with chronic liver conditions. The role focuses on improving treatment outcomes and reducing emergency department visits.

Key responsibilities

• Collaborate with multidisciplinary teams to develop goals, plan interventions, and maximize patient outcomes.
• Identify patients needing ongoing coordination, conduct outreach, and manage care plans using registries and patient lists.
• Perform comprehensive clinical assessments covering disease‑specific, medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Educate and coach patients and families on self‑management, monitor compliance, and update care plans.
• Serve as liaison and advocate, facilitating transitions of care and connecting patients to community resources.

Required profile

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience.

Required skills

• Utilize assessment tools and risk‑assessment instruments to identify care needs.
• Use technological tools such as registries, patient lists, and care team tabs for population management.
• Conduct comprehensive clinical assessments and develop individualized care plans.
• Provide patient education, coaching, and advocacy.
