About the role

We are seeking organized, reliable, and proactive Care Coordinators to join our team in Brooklyn, NY. This role supports home‑care operations by managing caregiver schedules and ensuring high‑quality service delivery for clients and families.

Key responsibilities

  • Coordinate, schedule, and monitor daily home care services and caregiver assignments.
  • Serve as the main point of contact for clients, families, and caregivers to handle inquiries and resolve operational changes.
  • Maintain accurate, up‑to‑date client files and care schedules in compliance with agency standards.
  • Prioritize multiple tasks efficiently in a fast‑paced environment.
  • Collaborate with internal teams to drive operational efficiency and high client satisfaction.

Required profile

  • Proven experience working in a home care setting and in a Care Coordinator role.
  • Exceptional organizational skills and strong attention to detail.
  • Excellent written and verbal communication skills with a proactive mindset.
  • Strong team player with a dependable, adaptable work ethic.
