Bilingual Housing Case Manager

About the role

The HRSN Housing Case Manager provides comprehensive, housing‑focused case management to individuals and families facing housing instability. This full‑time position supports participants receiving Health‑Related Social Needs funding by guiding them through housing searches, financial assistance, and long‑term stabilization.

Key responsibilities

• Assist participants in identifying safe, stable housing options and develop individualized stabilization plans.
• Engage landlords, property managers, and housing providers to secure placements and advocate for participants during lease negotiations.
• Coordinate financial assistance for rent, move‑in costs, utilities, and other eligible expenses while ensuring compliance with HRSN funding requirements.
• Maintain accurate case notes, documentation, and outcome tracking in HMIS or other required systems.

Required profile

• High school diploma or equivalent; strong organizational and communication abilities.
• Ability to maintain confidentiality and manage sensitive participant information.
• Valid driver’s license, proof of insurance, and reliable vehicle.
• Bilingual skills (Spanish, Russian, or other languages commonly spoken by participants) preferred.

Required skills

• Proficiency in Microsoft Office.
• Proficiency in Google Suite.
• Experience with client databases, preferably HMIS.
