Advanced Practice Provider – Palliative Medicine

About the role

The Cleveland Clinic Health System is seeking a qualified Advanced Practice Provider (NP, PA, or CNS) to join the Palliative Medicine Program at Mercy Hospital. The role involves inpatient consults, follow‑up visits, and multidisciplinary rounding to support adult patients with serious illness.

Key responsibilities

• Conduct new consults and follow‑up appointments, assessing symptoms and managing pain, nausea, and other distressing conditions.
• Lead serious‑illness conversations, establish goals of care, and coordinate discharge planning.
• Collaborate daily with physicians, nurses, social workers, and other team members during rounds.
• Provide weekend and holiday coverage as needed.

Required profile

• Current APRN (NP, PA, or CNS) licensure in the applicable state(s) with national certification.
• Graduate of an accredited APRN or Physician Assistant program; MSN required within 3 years of hire for external hires.
• Basic Life Support certification required; ACLS, PALS, or NRP may be required based on setting.
• ACHPN certification in Hospice and Palliative Care to be obtained within the first two years of employment.

Required skills

• Experience in oncology, ICU, or hospice settings (new graduates with extensive relevant experience may be considered).
• Ability to perform comprehensive medical histories, physical examinations, and order appropriate diagnostic tests.
• Prescribing authority in accordance with state board and Cleveland Clinic policies.
